New Patient Forms
When you schedule your first appointment, the forms you should bring with you will be emailed to you or a link texted to you.
In-Office Dental Savings Plans

Financial Policies
Accepted for payment are cash, checks, credit cards, CareCredit and LendingClub. We request payment when appointments for services are scheduled. While we are happy to help you with insurance claims and to direct you to third-party financing, please be aware that patients are responsible for payment of their charges.
Insurance
We will help you file claims to your insurance company and only require payment of your potion of the cost of treatment when you schedule your appointment.
